We offer case management services to seniors and individuals with physical disabilities who participate in the EPD Waiver program in Washington DC. Our case managers are social workers and nurses with the knowledge and experience to coordinate your care needs and connect you with the resources in the community. Some of the services available to EPD Waiver beneficiaries are:
- Personal Care Aide (PCA)
- Respite services (PCA services to relieve caregiver)
- Personal Emergency Response System (PERS)
- Skilled Services (PT, OT, SL, SN)
- Homemaker Services
- Chore Aide Services
- Assisted Living Facility
- Adult Day Health Program (ADHP)
- Service My Way (SMW)
